Job summary

ScionHealth’s Kindred Hospital Chicago - Northlake is seeking a Wound Care Nurse to provide specialized wound, ostomy, and skin care. The role includes assessment, care planning, treatment implementation, education, and meticulous documentation in line with clinical standards.

You will work within a multidisciplinary team to optimize patient outcomes and reduce skin breakdown risks. The position requires active RN licensure, WOCN/CWS/WCC certification within 12 months, BCLS adherence, and 2+

Qualifications

RN license in state of practice required at hire.
WOCN/CWS or WCC certification within 1 year of hire required.
BCLS (Basic Life Support) within facility-specified time frame on hire.

Responsibilities

Provide comprehensive wound assessments and collaborate with providers on treatment plans.
Implement and monitor wound care interventions including negative pressure therapy and dressing changes.
Coordinate care for patients with complex wounds and ensure continuity through discharge planning.
Educate patients, families, and staff on wound care techniques and prevention.
Identify patients at risk for skin breakdown and implement prevention protocols.
Serve as clinical resource for skin integrity and wound management.
