Overview

Kindred Hospital Chicago - North is a 164‑bed long‑term acute care hospital offering in‑depth care similar to a traditional hospital, but for an extended recovery period. We partner with physicians and provide 24‑hour clinical care seven days a week to support patients’ wellness journey. The facility is located along the North Branch Chicago River and surrounded by the greenery of Horner, Welles, and Revere Parks.

Job Summary

The wound care coordinator manages the facility’s wound care program, establishes standards of care, and provides direct patient care and assistance to staff nurses. The role also acts as a consultative service to affiliated healthcare agencies.

Essential Functions
  • Develops and implements the wound care program, including patient care protocols, documentation tools, the wound care formulary, and referral criteria.
  • Establishes standards of care, competencies, policies, and procedures to provide quality, cost‑efficient, and effective wound care for all clinical staff.
  • Acts as primary consultant to wound care clinicians and provides training and support to enable staff to assess wounds and create plans of care.
  • Provides ongoing education to staff on available products and best practices.
  • Evaluates all wounds upon admission and during treatment to determine a plan of care and identify problems early.
  • Provides consultation for patients with pressure injuries; consults on wounds not showing measurable healing within two weeks.
  • Assists the primary nurse in developing appropriate plans in collaboration with physicians.
  • Recommends changes to wound care orders and supplies evidence‑based research support as needed.
  • Reviews medical records to ensure accurate documentation of wound assessments, care plans, and treatments.
  • Investigates adverse events through root‑cause analysis and develops action plans with nursing leadership.
  • Participates in clinical outcome monitoring, follow‑up, and performance improvement initiatives.
  • Contributes to CMS quality data reporting by completing wound‑related LTRAX data records.
  • Collaborates with Clinical Operations staff to maintain a cost‑effective wound care formulary.
  • Assists with training of new hire wound care coordinators within the hospital division.

  • Knowledge of wound care consistent with NPIAP, AHCPR, WOCN, and CDC guidelines.
  • Knowledge of wound debridement techniques appropriate to scope of practice.
  • Ability to serve as a resource to nursing staff for complex wound management.
  • Leadership, motivation, and development of individuals and teams.
  • Program management skills.
  • Proficient reading, writing, and speaking in English.
  • Demonstrates regular attendance and reliability.

Pay Range: $37.00–$54.98 per hour.
