Job summary

Apploi is seeking a Wound Care Nurse (RN) to provide specialized wound management for residents, with a focus on safety, precise documentation, and evidence-based treatments. You will assess wounds, develop individualized care plans, and collaborate with the interdisciplinary team to optimize healing outcomes.

The role requires a BSN or equivalent RN program completion, an active RN license, CPR/BLS, and at least two years of clinical nursing experience, preferably with geriatrics.

Qualifications

• BSN preferred; graduate of an accredited RN program required.
• Current, active RN license.
• CPR/BLS certification required.
• Minimum of two (2) years of clinical nursing experience.
• Geriatrics or subacute experience preferred.
• Management or leadership experience a plus.

Responsibilities

• Maintain accurate and updated care plans for all skin impairments.
• Conduct weekly wound rounds with the wound consultant.
• Perform and document weekly wound measurements with accuracy and timeliness.
• Monitor and evaluate all new pressure-related skin openings and determine root causes.
• Assess and manage surgical, arterial, venous, and other complex wounds.
• Ensure all treatments, changes, and follow-up needs are documented thoroughly.
• Educate residents, families, and caregivers on wound care procedures and expectations.
• Demonstrate wound care techniques to caregivers to support staff competency.
• Communicate with the interdisciplinary team to support coordinated care.
