Job summary

Autumn Care of Suffolk is recruiting an RN/LPN to assess residents’ needs, craft individualized care plans, administer nursing care, and supervise nursing staff in a supportive community setting in Suffolk, VA. You will ensure adherence to applicable standards and policies while delivering high-quality resident care.

The successful candidate will have an active RN or LPN license in Virginia, a current CPR credential, and training in infection prevention.

Qualifications

• Must hold a current, unencumbered RN or LPN license in the state of employment.
• Must hold a current CPR certification.
• Must complete or have completed infection prevention and control training (CDC modules or equivalent).
• Must be able to communicate in English and follow verbal/written instructions.
• Ability to work hours as scheduled.

Responsibilities

• Assess residents’ needs and develop individualized care plans.
• Administer nursing care and evaluate care outcomes.
• Supervise nursing assistants and other personnel per regulations.
• Maintain compliance with federal, state and local standards and facility policies.
