Brown's Health and Rehab is seeking a skilled and compassionate Wound Care RN to join our clinical team. The RN will assess, plan, implement, and evaluate wound care plans for residents in collaboration with the interdisciplinary team.

Responsibilities include wound assessments, dressing changes, debridement, wound vac management, monitoring healing, EMR documentation, and education of staff. Collaboration with physicians and therapists ensures holistic care. Georgia licensure required.

Qualifikationen

• Current and valid RN license in Georgia.
• Wound care certification preferred or willingness to obtain.
• Minimum 1-2 years of nursing experience in long-term care or rehabilitation; wound care experience strongly preferred.
• Strong clinical assessment skills and attention to detail.
• Excellent communication and documentation skills.

Aufgaben

• Provide wound assessments and develop individualized treatment plans.
• Administer wound care treatments including dressings, debridement, and wound vac management.
• Monitor healing progress, document outcomes, and update care plans accordingly.
• Educate staff on wound prevention, identification, and care.
• Collaborate with physicians, dietitians, therapists, and other team members for holistic wound management.
• Maintain accurate EMR documentation.
• Ensure compliance with regulations and facility policies.
• Participate in wound rounds and QAPI initiatives.
• Provide training to CNAs and LPNs on skin integrity protocols and pressure ulcer prevention.
• Assist DON and Unit Manager with taking call.
