Job summary

Evergreen Center for Health & Rehabilitation is seeking a Wound Care Nurse to assess, treat, and monitor wound conditions in collaboration with the interdisciplinary team. You will develop care plans and perform dressing changes, debridement, and irrigation to improve healing and residents’ quality of life.

Qualifications

• Valid state RN nursing license.
• Wound Care Certified preferred.
• Degree in nursing or related field preferred.
• 2+ years in wound care management, preferably in long-term care.
• CWCN Certification preferred.

Responsibilities

• Provide oversight and expertise of wound care and prevention program for residents and staff.
• Conduct comprehensive wound assessments and develop individualized care plans for patients with skin complications.
• Provide direct wound care interventions, including dressing changes, debridement, and wound irrigation.
• Review, update and direct the implementation of wound care & prevention policies and procedures.
• Monitor wound healing progress and evaluate treatment effectiveness.
• Educate residents, families, and staff on wound care protocols and prevention strategies.
• Collaborate with physicians and wound care specialists to coordinate care.
• Maintain accurate documentation of wound assessments, interventions, and outcomes.
