Job summary

Edgefield Post Acute is a skilled nursing and rehabilitation facility in Edgefield, SC, seeking an experienced RN Wound Nurse to join a compassionate interdisciplinary team. You will assess, treat, and monitor wounds, educate residents and families, and contribute to comprehensive care plans.

The role emphasizes wound care expertise, accurate documentation, and collaboration with physicians, DON, and therapy staff to improve residents’ quality of life in a supportive environment.

Qualifications

• Must have wound care experience.
• Must be an active RN in good standing.
• Minimum of 2 years nursing experience.
• Excellent assessment, documentation, and communication skills.
• Strong clinical judgment and critical thinking.

Responsibilities

• Assess, manage, and treat wounds, pressure injuries, skin tears, ulcers, and other dermatologic conditions.
• Complete comprehensive skin assessments and quarterly dermatologic evaluations.
• Collaborate with physicians, DON, therapy, and inter-disciplinary team to develop individualized plans.
• Provide accurate written and verbal reports on resident progress and wound healing.
• Monitor wound healing and update care plans as needed.
• Educate residents and families on wound prevention and treatment plans.
• Document thoroughly while ensuring HIPAA compliance.
