Responsibilities

• Actively manages patient care correlating initial assessment, treatment course, and other services.
• Communicates patient care effectively.
• Calls physicians/offices to clarify physician orders.
• Assess data reflective of the patient's status and interprets the information needed to identify educational needs.
• Ensures wound/ostomy/skin plan of care is understood by the continuum of care and coordinates with Home Health, DME Company, and agencies.
• Documents patient's response to interventions.
• Performs patient assessments and teaching adapted to age-specific needs.
• Considers special patient needs when delivering nursing care.
• Evaluates patient/family cultural and cognitive needs and adjusts education accordingly.
• Performs bedside lab testing and quality control per orders and lab procedures.
• Provides wound care education and documentation; stays updated on wound/skin care trends.
• Maintains knowledge of skin care, wound and ostomy topics.
• Demonstrates nursing care for procedures and treatments.
• Provides discharge instructions and patient education.
• Assists in product evaluation for ostomy and wound/skin care products.
• Collaborates with staff nurses and uses the nursing process to plan and evaluate teaching.
• Documents teaching efforts and outcomes.
• Keeps abreast of industry trends for professional growth.
