Job summary

Woodland Grove Health & Rehab in Jacksonville, FL is seeking a Wound Care (RN) to join our caring team. The facility offers a quiet, relaxing environment with a focus on personalized resident care and rehabilitation services.

The Wound Care RN will assess wounds, plan ongoing care, educate residents and families, supervise CNAs, and contribute to wound management policies to ensure quality outcomes.

Qualifications

• Must be a RN in good standing and licensed by FL.
• Minimum of one year experience in long-term care setting.
• Must possess leadership and supervisory ability and willingness to work harmoniously with staff.

Responsibilities

• Perform wound assessments, reassessment, and management for all residents.
• Create plan of care including goals, treatment, evaluation and outcomes of residents.
• Provide education to residents, family, and staff regarding wound management, prevention, support, and other therapies.
• Direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs and other nursing personnel.
• Demonstrate knowledge and skill to develop and implement policies and procedures to manage residents with wounds.
