Job summary

Carlton Health & Rehab in Daytona, FL is seeking a Wound Care RN/LPN to join our caring team. You will perform wound assessments, develop plans of care, educate residents and families, and supervise day-to-day nursing activities with CNAs.

Must hold an active RN or LPN license in Florida, have at least one year of long-term care experience, and demonstrate leadership and teamwork. This full-time role offers a supportive work environment and opportunities for continuing education.

Qualifications

  • Must be RN or LPN in good standing and currently licensed by the state (FL).
  • Minimum of one year experience in long-term care setting.
  • Must be a supportive team member, contribute to, and be an example of teamwork.
  • Demonstrate leadership and supervisory ability and willingness to work harmoniously with staff.

Responsibilities

  • Perform wound assessments, reassessment, and management for all residents.
  • Create plan of care including goals, treatment, evaluation and outcomes.
  • Provide education to residents, family, and staff regarding wound management and prevention.
  • Direct nursing care to residents, and supervise day-to-day nursing activities performed by CNAs and other nursing personnel.
  • Demonstrate knowledge and skill to develop and implement policies and procedures to manage residents with wounds.
