Qualifications

• Valid state RN nursing license.
• 2+ years in wound care management, preferably in Long-Term Care.
• CWCN certification preferred.

Responsibilities

• Provide oversight and expertise of wound care and prevention program for residents and staff.
• Conduct comprehensive wound assessments and develop individualized care plans.
• Provide direct wound care interventions, including dressing changes, debridement, and wound irrigation.
• Review, update and direct the implementation of wound care & prevention policies and procedures.
• Monitor wound healing progress and evaluate treatment modalities.
• Educate residents, families, and staff on wound care protocols and prevention strategies.
• Collaborate with physicians and wound care specialists to coordinate care.
• Maintain accurate documentation of wound assessments, interventions, and outcomes for regulatory compliance.
