Job summary
Hebrew Center for Health & Rehabilitation in West Hartford is seeking a Wound Care Nurse to assess, treat, and monitor wound conditions in collaboration with the interdisciplinary team. You will develop care plans and provide direct wound care to improve healing and residents' quality of life.
The role requires current RN license, 2+ years wound care experience, and CWCN certification preferred; you will educate staff, maintain documentation, and coordinate with physicians, with opportunities
Qualifications
- RN license in good standing
- Degree in nursing (BSN or higher) or related field preferred
- Minimum 2+ years in wound care management, ideally in long-term care
- CWCN certification preferred if available
- Strong clinical assessment, documentation, and communication skills
Responsibilities
- Oversee wound care and prevention program for residents and staff
- Conduct wound assessments and develop individualized care plans
- Provide direct wound care interventions (dressings, debridement, cleansing)
- Review/update wound care policies and procedures
- Monitor healing progress and evaluate treatment effectiveness
- Educate residents, families, and staff on wound care protocols
- Collaborate with physicians and wound care specialists to coordinate care
- Maintain accurate documentation for regulatory compliance and reimbursement
