Job Description:

As a Wound Care Nurse, you will play a crucial role in ensuring the highest standard of care for patients with wounds. You will collaborate with healthcare professionals to develop and implement personalized wound care plans, monitor progress, and educate patients and their families on proper wound management techniques.

Responsibilities:

• Assess and evaluate patients with wounds, identifying individual needs and developing customized care plans.
• Coordinate and communicate with healthcare providers to ensure seamless and effective wound care.
• Monitor and document wound healing progress, adjusting care plans as needed.
• Educate patients and their families on proper wound care techniques, prevention strategies, and lifestyle modifications.
• Collaborate with interdisciplinary teams to enhance patient outcomes.
• Stay updated on the latest advancements in wound care and implement best practices.

Qualifications:

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid state IL license.
• Certification in Wound Care (WCC) is highly desirable.
• Excellent communication and interpersonal skills.
• Strong organizational and problem-solving abilities.
• Passion for delivering compassionate and patient-centered care.
