Description:

The Wound Care Nurse is responsible for providing primary skin care to residents under the medical direction and supervision of the residents’ attending physician, the Director of Nursing, or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders.

Responsibilities for Wound Care Nurse:
  1. Confer with the Director of Nursing and/or other licensed nursing personnel regarding the dermatologic disorders of residents in the facility.
  2. Examine the resident and their records, and distinguish between normal and abnormal findings to recognize when to refer the resident to a physician for evaluation, supervision, or directions.
  3. Provide assessment and diagnostic services to residents using observation, inspection, and palpation.
  4. Initiate requests for consultation or referral. Respond to requests from residents, physicians, or nursing staff.
  5. Identify, manage, and treat skin disorders and lesions, such as abrasions, foot problems, decubitus ulcers, infections, scaling diseases, and benign tumors.
  6. Assist in resident rehabilitation activities, perform range of motion exercises, assist in ambulation, provide emotional support, and monitor physical therapy results.
  7. Provide preventative health care advice, including nutrition and skin disorder prevention.
  8. Report in writing and verbally to physicians, the Medical Director, or the DON regarding residents' status and care.
  9. Implement and interpret policies and procedures related to skin care treatments to physicians, residents, families, and the public.
  10. Perform other duties as assigned.
Qualifications for Wound Care Nurse:
  • LPN / RN with current Illinois state licensure (Required).
  • Wound Certification from an accredited institution preferred.
  • Minimum of two (2) years of experience as an LPN / RN.
