All American Village Nursing and Rehabilitation is seeking a dedicated Wound Care and Infection Prevention Nurse to join our team. This dual-role position is crucial to ensuring resident safety and quality of care. The ideal candidate will have experience in both wound management and infection control, with a passion for clinical excellence and resident-centered care.
Wound Care Duties:
Assess, document, and monitor wounds using evidence-based protocols.
Administer wound care treatments, including dressings, debridement, and negative pressure therapy.
Collaborate with physicians, NPs, and interdisciplinary team members to optimize wound healing.
Educate staff on wound care best practices and preventive measures.
Track wound healing outcomes and participate in QAPI initiatives.
Infection Prevention Duties:
Develop, implement, and monitor the facility’s Infection Prevention and Control Program (IPCP).
Conduct infection surveillance, tracking, and reporting per CDC and state/federal guidelines.
Educate staff on infection prevention protocols, hand hygiene, PPE, and outbreak response.
Lead investigations and reporting for any infection outbreaks or communicable disease exposures.
Ensure compliance with CMS, OSHA, CDC, and state public health regulations.
Licensed RN or LPN in the state of IL
Wound Care Certification (WCC, WOCN, or similar) required.
Infection Preventionist (IP) Certification (CDC, APIC, or equivalent) or willingness to complete within 6 months of hire.
Minimum 1 year of wound care and/or infection control experience in a SNF or long-term care setting.
Strong clinical assessment, communication, and documentation skills.
Knowledge of CMS infection control regulations and SNF documentation practices.
