Job summary

FutureCare seeks a seasoned Wound Care Nurse to oversee wound care practices and drive skin health across our resident population. You will lead rounds, collaborate with the care planning team, and tailor treatment plans within established protocols.

Responsibilities

• Oversee all direct wound care activities to ensure practices are consistent.
• Identify, plan, implement, and evaluate skin care in residents.
• Manage and treat specific skin disorders.
• Coordinate weekly skin rounds.
• Perform assessments using observation and inspection.
• Collaborate with Care Planning Team to develop care plans.
• Provide treatment for skin conditions within wound care protocol.
• Monitor preventive measures such as inspection, turning and positioning.
• Precept new nurses and demonstrate procedures to new staff.
• Order and track supplies and equipment for prevention and treatment.
• Modify treatment regimens to meet resident needs per medical guidelines.
• Educate residents and caregivers on prevention techniques and health maintenance.
• Participate in Performance Improvement programs and wound/nutrition committees.

• Confer with the Care Planning Team in the development of the care plan
• Provide treatment forskin disorders within established wound care protocol
• Monitor the use of preventive measures such as inspection, turning and positioning
• Precept new nurses and demonstrate individual procedures to new nursing personnel
• Order and tracks the usage of supplies/equipment in prevention and treatment
• Assist in modifying the treatment regimen to meet the needs of the resident, in accordance with established medical practices, the regulatory guidelines and the policies and goals of this facility
• Provides information to resident and caregivers on prevention techniques and other ways to maintain or improve the general health and psychosocial status of the resident
• Participates in Performance Improvement program as assigned such as wound and nutrition committee
• Participate in the planning and conducting of in-service training classes concerning dermatologic nursing skills and procedures used within the facility
• Abide by the standards identified in FutureCare's Statement of Corporate Ethics and the Corporate Compliance Plan

Qualifications

• Must hold an active RN license issued in the state of Maryland
• At least 1 year of nursing experience required
• Must possess an active certification in wound care
• Experience with resident's skin care working in a skilled nursing facility preferred
• Proven leadership skills to assure smooth unit/area operations and quality resident care
• Effective communication skills to facilitate positive interpersonal interaction
• Ability to interact with others in a considerate, helpful, and courteous manner
