Job summary

CareMed Health Solutions is seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities in West Virginia. You will assess, diagnose, and treat complex wounds and collaborate with an interdisciplinary care team.

Requirements include current WV NP license, board certification, optional DEA, and experience with wound care; the role includes prescribing medications, ordering tests, and coordinating with facility staff to ensure optimal patient

Qualifications

Active NP license in West Virginia.
Board certification in a relevant specialty.
DEA licensure (optional).
Experience with wound care in long-term care.
Comfort diagnosing and treating diabetic, arterial, venous, and Kennedy ulcers.
Experience with wound vacs and advanced wound care products.
Familiar with EMR and interdisciplinary care teams.

Responsibilities

Provide wound care services for long-term care patients residing in skilled nursing facilities.
Prescribe medications, including controlled substances in accordance with WV Law.
Order, review, and follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular basis to discuss changes in condition and the care plan.
Collaborate with the primary care physician.
