Job summary

CareMed Health Solutions is seeking a Wound Nurse Practitioner to deliver wound care to residents in long-term care facilities in West Virginia. The role emphasizes collaboration with an interdisciplinary team and primary care physicians to optimize resident outcomes.

Must hold an active WV NP license with board certification; DEA license is optional. Experience with wound care and EMI systems in a facility setting is preferred.

Qualifications

• Must hold a current, unrestricted NP license in West Virginia.
• Board certification required.
• DEA licensure optional.
• Clinical background in adult, family, or geriatric care.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per WV law.
• Order, review, and follow up on treatments and diagnostic testing.
• Communicate with nursing home staff regarding changes in condition and care plans.
• Collaborate with the primary care physician.
