• Perform wound assessments, reassessments, and ongoing management.
• Develop and implement individualized wound care plans.
• Monitor wound progress and document treatments and outcomes.
• Identify changes in wound condition and communicate with the medical team.
• Collaborate with physicians, nurses, CNAs, and other care team members.
• Support infection prevention and control across the facility.
• Assist with infection prevention data and staff education.
