Job summary

Saint Francis Hospital, part of Trinity Health Mid-Atlantic, is seeking an experienced Registered Nurse to join our wound care team. This part-time role supports care for in-patients and out-patients, with a schedule Monday, Wednesday, and Friday from 8am to 4pm.

The wound care nurse will conduct detailed exams, manage wounds, educate patients and families, collaborate with physicians and therapists, and document care plans and clinical pathways to ensure quality outcomes.

Qualifications

• Current licensure as a Registered Nurse in the State of Delaware required.
• ASN required, BSN preferred, from an accredited school of nursing.
• At least 3 years' of clinical experience required, preferably in Wound Care.
• Wound Care certification preferred; will consider those with experience and able to obtain certification within 6 months of hire.

Responsibilities

• Adheres to hospital nursing practice standards, demonstrates appropriate history and physical assessment, planning, implementation and evaluation of patient care.
• Implements appropriate Nursing actions to each patient that is identified.
• Thorough History and Physical’s will be done on every patient in the wound center.
• Makes decisions that reflect both knowledge of facts and sound judgment.
• Discusses with and includes patient and/or family significant other about planned patient care and expected outcomes and documents it.
• Identifies and appropriately utilizes multidisciplinary approach to facilitate delivery of patient care.
• Evaluates the patient’s/significant other’s understanding of instructions.
• Gives written instructions and materials to patients and families.
• Reviews preventative health measures to patient and family. Documents understanding of instructions.
• Completes all charts/forms accurately and timely.
• Receives and gives a knowledgeable and concise patient report to others, as necessary to demonstrate awareness of patient’s overall condition.
• Initiates Clinical Pathways/Plan of Care on appropriate patient population at patients first visit.
• Updates and initiates Care Plans and Pathways as changes in patient’s needs are assessed.
