Integrated Wound Care is seeking a compassionate Nurse Practitioner to join our team on a part-time basis. We deliver quality wound care to skilled nursing, long-term care, and rehabilitation facilities, aiming to reduce hospitalizations and improve patient outcomes.

Your role involves conducting wound assessments, developing personalized treatment plans, and collaborating with a multidisciplinary team to ensure optimal patient care.

Qualifications

• Active license in state of employment.
• Minimum of 1 year of wound care experience.
• Ability to work independently and as part of a team.
• Excellent verbal and written communication skills.
• Comfortable with technology and electronic medical records.

Responsibilities

• Conduct wound assessments and provide personalized treatment plans for patients.
• Work collaboratively with a team of healthcare professionals to ensure optimal patient care.
• Complete documentation accurately and in a timely manner.
• Educate patients and their families on wound care, prevention, and management.
• Monitor patient progress and adjust treatment plans accordingly.
