Job description

Join to apply for the Wound Care Nurse role at Empire Care Centers.

We are currently seeking a full-time wound care RN/LPN. The wound care nurse participates in assessing and evaluating the clinical needs of residents with wounds, and works with the residents and the care team to plan and implement appropriate care. This role supports the mission of Woodstock Nursing and Rehabilitation Center.

Hours: Full-time, Monday-Friday (8am-4pm), includes every other weekend.

Wound Care Nurse Responsibilities:

1. Conduct comprehensive wound consultations, including detailed assessments and recommendations for interventions and care planning.
2. Perform wound assessments for residents and complete all documentation, tracking, and care planning activities.
3. Monitor all assigned residents with wounds regularly.
4. Assess residents'/caregivers' knowledge of wound prevention and treatment; provide education on prevention and care as needed.
