Certified Wound Care Nurse Practitioners

Location: Arizona, United States

Wound Care Health Group is seeking experienced Certified Wound Care Nurse Practitioners (NP-C, FNP, or AGNP) with specialized certification in wound, ostomy, and/or continence care (CWCN, CWS, WOCNCB) to join our mobile wound care team. This role involves conducting in-home visits, assessing wounds, developing treatment plans, and collaborating with interdisciplinary teams.

Position Details

• Type: Part-time

Pay Range

Base pay: $85.00/hr - $125.00/hr. Actual pay depends on skills and experience. Mileage reimbursement and flexible schedule offered.

Key Responsibilities

• Conduct comprehensive wound assessments during visits
• Perform wound debridement (enzymatic, sharp, autolytic, etc.)
• Develop and update individualized care plans
• Order and interpret labs, imaging, diagnostics
• Collaborate with healthcare team members
• Document visits in EMR system
• Educate patients and caregivers on wound care
• Participate in quality improvement initiatives

Qualifications

• Wound care certification: CWCN, CWS, WOCNCB, or equivalent
• 2+ years experience in wound care or home health preferred
• Valid driver’s license and reliable transportation
• Strong clinical judgment and decision-making skills
• Proficient with EMR and mobile documentation
• Continuing education & certification reimbursement
