  • 20 Hours/Week
  • Shift Start Times Vary - 11AM - 7:30PM and 12PM - 8:30PM
  • Occasional weekends - either Saturday or Sunday shifts

Job Description

The Wound Care Certified Nurse reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory standards.

The nurse provides consultation and direct assistance in managing patients with or at risk for altered skin integrity, including complicated wounds, pressure injuries, incontinence, and ostomy management. The role involves collaboration with the healthcare team, patient education, and developing individualized care plans.

Responsibilities include:
  • Delivering expert wound care and coaching to the interdisciplinary team.
  • Providing consultative services for wound care planning and delivery.
  • Assisting with direct patient care for skin breakdown prevention and treatment.
  • Collaborating on care plans to promote healing.
  • Partnering with referral liaisons and leadership to ensure smooth care transitions.
  • Assessing patient/family needs and formulating treatment plans.
  • Documenting assessments and recommendations.
  • Developing wound care product formulary in partnership with vendors and staff.
Education & Quality Improvement
  • Providing education to new staff and physicians on wound care guidelines.
  • Developing patient educational materials and updating policies.
  • Leading quality improvement initiatives related to wound care and skin injury surveillance.
Qualifications

Required:

  • Licensed as a professional nurse in Illinois.
  • Bachelor's Degree.
  • Current certifications: WCC, CWCN, CWON, or WOCN.
  • Strong communication skills.

Preferred:

  • 3-4 years of rehabilitation nursing experience.
  • MSN preferred.
