Woundtech in Jacksonville, FL is seeking a Nurse Practitioner or Physician Assistant to perform home-based wound care visits and manage patient progress. You will document visits promptly and collaborate with supervising physicians to ensure optimal healing outcomes.

Qualifications

• Graduate nursing degree or PA credential with active licensed status.
• National certification as NP or PA; wound care certification preferred.
• 1–3 years at advanced practice level; autonomous practice expected.

Responsibilities

• Provide hands-on care to patients during home visits per needs.
• Debride wounds by sharp, enzymatic, autolytic or surgical methods.
• Apply topical wound care and related products.
• Recommend services to improve wound healing, including tests and referrals.
• Interpret diagnostics and follow up appropriately.
• Prescribe medications and DME within scope and supervision.
• Assess diabetes risks per ADA guidelines.
• Educate patients and coordinate with PCP/CM/physicians.
• Update PCPs and other clinicians on patient status.
