Responsibilities include in-person and telehealth visits, CMS-compliant documentation, and collaboration with clinical teams to optimize patient outcomes. Requires Masters, RN license, and active wound care certification; travel and HIPAA compliance are expected.

Qualifications

• Current Certification in Wound Care is required.
• Preferred CWOCN or CWCN certification.
• Alternative: Wound Care Certified or Certified Wound Specialist with 3+ years in wound care.
• Minimum 3 years full-time experience as a wound specialist in acute care, SNF, home health, or outpatient, plus 1 year as a Nurse Practitioner in a wound-related role.

Responsabilités

• Provide in-person evaluation and management visits for wounds in home settings, assisted living, and skilled nursing facilities; debridement and procedures per CMS guidelines.
• Provide telehealth or digital monitoring visits for wound patients; virtual care up to 20%.
• Follow infection prevention protocols and proper handling of equipment; ensure CMS-compliant documentation.
• Document services in real time via EMR; complete billable documentation within 1 business day.
• Collaborate with other clinicians to coordinate care and optimize outcomes.
• Lead in quality improvement initiatives and participate in program development for the nurse practitioner wound program.
