Job summary

Avir at Pearland in Pearland, TX is seeking a compassionate Wound Care Nurse (LVN) to join our care team. You will assess wounds, provide dressing changes, debridement, and irrigation, and coordinate with physicians and therapists to promote healing and comfort.

Current Texas LVN license is required. You will educate patients and families, maintain accurate records, and participate in quality improvement initiatives to advance patient care.

Qualifications

• Current LVN licensure in Texas.
• 0+ years wound care experience preferred.
• Ability to work independently and in a team.
• Strong communication and interpersonal skills.
• Proficient in electronic medical records (EMR)

Responsibilities

• Assess patients' wound care needs and develop individualized treatment plans to promote healing, prevent infection, and alleviate pain.
• Conduct wound care treatments, including dressing changes, debridement, and irrigation.
• Monitor patients' progress and adjust treatment plans as needed to ensure optimal outcomes.
• Collaborate with interdisciplinary teams, including physicians, physical therapists, and occupational therapists, to ensure comprehensive care.
• Educate patients and their families on wound care and prevention techniques.
• Maintain accurate and detailed records of patient care and treatment plans.
• Participate in quality improvement initiatives to improve patient outcomes and streamline processes
