Job summary

Avir at Skyline Park is seeking an LVN Wound Care professional to join our warm, home-like setting. You will perform wound assessments, develop and implement individualized care plans, and work with an interdisciplinary team to support patient recovery.

The role requires Texas LVN licensure, ability to stand for extended periods, and strong communication with patients and families. We offer ongoing education, professional development, and a competitive compensation package.

Responsibilities

• Provide wound assessments and develop treatment plans with the interdisciplinary team

• Educate patients and families on wound care techniques
• Collaborate with physicians, occupational therapists, and physical therapists to ensure seamless care
• Participate in quality improvement initiatives for wound care services

Responsibilities:

• Provide high-quality patient care, conducting wound assessments, and developing and implementing individualized treatment plans in collaboration with the interdisciplinary team
• Perform wound debridement, dressing changes, and other wound care procedures under the supervision of the RN and Wound Care Specialist
• Monitor and document patient progress, reporting any changes or concerns to the Wound Care Specialist and RN
• Educate patients and their families on wound care techniques, dressing changes, and self-care instructions
• Collaborate with other healthcare professionals, including physicians, occupational therapists, and physical therapists, to ensure seamless care
• Participate in quality improvement initiatives to ensure our wound care services meet the highest standards of excellence

Requirements:

• Currently licensed as an LVN in the state of Texas
