Job summary

Avir at Skyline Park in Texas is seeking an LVN Wound Care professional to join our compassionate team. You'll perform wound assessments, develop individualized plans, and collaborate with RNs and therapists in a warm, home-like setting.

You will debride wounds, change dressings, document progress, educate patients and families, and participate in quality improvements to ensure the highest standards of care. Texas LVN license required; experience in wound care preferred.

As an LVN Wound Care professional, you will play a vital role in helping our patients regain strength, confidence, and independence in a warm, home-like setting that feels safe and supportive.

Responsibilities:

• Provide high-quality patient care, conducting wound assessments, and developing and implementing individualized treatment plans in collaboration with the interdisciplinary team
• Perform wound debridement, dressing changes, and other wound care procedures under the supervision of the RN and Wound Care Specialist
• Monitor and document patient progress, reporting any changes or concerns to the Wound Care Specialist and RN
• Educate patients and their families on wound care techniques, dressing changes, and self-care instructions
• Collaborate with other healthcare professionals, including physicians, occupational therapists, and physical therapists, to ensure seamless care
• Participate in quality improvement initiatives to ensure our wound care services meet the highest standards of excellence

Requirements:

• Currently licensed as an LVN in the state of Texas
