Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking a compassionate Wound Care LPN to join our skilled nursing team. You will provide specialized wound care, assess wounds, and collaborate with physicians and nursing staff to optimize resident outcomes.

Qualifications

• Current and active New York LPN license.
• Wound care experience in a skilled nursing facility or long-term care setting preferred.
• Wound care certification is a plus.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Perform wound assessments and provide treatment in accordance with physician orders and facility protocols.
• Monitor and document wound progress and communicate changes to the interdisciplinary team.
• Assist in developing and implementing individualized wound care plans.
• Ensure accurate and timely documentation of all treatments and assessments.
• Educate residents, families, and staff on wound prevention and treatment strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with all federal, state, and facility regulations and standards.
• Collaborate with physicians, nurse practitioners, and nursing staff to ensure excellent resident outcomes.
