Job summary

Healogics, Inc. is seeking a Licensed Practical Nurse (LPN) to join our Melrose Park wound care team on a part-time, PRN basis. Under RN supervision, you will deliver wound care, assist in vein clinics, and support intake and discharge activities.

The role requires an IL LPN/LVN license, a high school diploma or GED, and at least 2 years of LPN/LVN experience, preferably in SNF or LTACH settings. Strong communication and multitasking are essential.

Qualifications

• High school diploma or GED required.
• Graduate from an accredited practical nursing program/state approved education program.
• Current LPN/LVN license in IL and ability to maintain it while employed.
• Two+ years of LPN/LVN experience, including SNF or LTACH preferred.

Responsibilities

• Provide wound care under RN/Provider guidance per policies and procedures.
• Assist in vein clinic and after Vein Clinic training.
• May assist with intake, discharge, or case management under RN direction.
• Collect patient information and prepare necessary equipment.
• Document and communicate care concerns with the care team.
• Perform hyperbaric-related duties after training, as assigned.
