Job summary

Healogics, LLC is seeking a Wound Care Center LPN for our Florida sites. Under RN/Provider supervision, you will deliver wound-related nursing care, follow hospital and CMS guidelines, and contribute to high-quality patient outcomes.

Your duties include assisting in vein clinic, intake and discharge support, case management with RN guidance, and maintaining accurate clinical documentation. A valid LPN/LVN license and 2+ years of experience are required.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice.
• Two or more years’ experience as LPN/LVN, including SNF/LTACH setting preferred.
• Wound care experience preferred.

Responsibilities

• Under RN/Provider supervision, provides wound-related patient care per protocols.
• Assist in vein clinic as trained after Vein Clinic training completed.
• May assist with intake and discharge.
• May provide case management assistance under RN guidance.
• Gathers patient information and necessary equipment.
• Reports quality of care issues to Clinical Nurse Manager.
• Communicates with team regarding therapeutic interventions.
• Performs hyperbaric therapy-related duties after training.
• May serve as a Documentation Assistant as needed.
• Performs other duties as delegated.
