Job summary

Qualifications

• Doctorate/masters/bachelor in Physical Therapy from CAPTE-accredited program.
• Current license from your state Board of Physical Therapy Examiners.
• Minimum 3 years’ experience as a Physical Therapist.

Responsibilities

• Work as part of care continuum with physicians, PAs/NPs, OT, health coach, and others.
• Administer PT treatments including exercise, activities, manual therapies, and patient education.
• Plan and execute goal-directed PT programs based on assessments.
• Assist with care coordination and follow-up care as needed.
• Teach in-home therapeutic exercises and maintain documentation in EMR.
• Maintain infection-control policies and a safe work environment.
• Develop staff through training and evidence-based practices.
