Job summary

The Society of St. Vincent de Paul - Phoenix is seeking a Workforce Case Manager to bridge guests with ongoing services, advocate for their needs, and remove barriers to progress for those experiencing homelessness.

You will map out plans, assist with resumes, soft-skill development, and job/housing searches. The role requires teamwork with staff and volunteers, quick thinking in emergencies, and clear communication to help guests move toward stability.

Qualifications

• Bachelor’s degree preferred; HS diploma or GED required.
• Minimum 1+ years Case Management or 2+ years in human services.
• Experience with vulnerable populations; trauma-informed & strengths-based support.
• Excellent communication and organizational skills.
• Ability to work independently and as part of a team.
• Valid driver’s license preferred.

Responsibilities

• Carry a caseload of workforce participants, meeting with each resident at least weekly.
• Actively recruit new participants and assess needs and eligibility for workforce programs.
• Develop individualized service plans with clients addressing needs, goals, and barriers.
• Monitor progress toward stabilization, income, and housing goals with case notes and plans.
• Maintain timely records in data systems within 24 hours of interaction.
• Coordinate with staff on incident intervention and conflict resolution as needed.
