Job summary

Wraparound Maryland, Inc. is seeking energetic Case Managers to advocate for adults and families in Worcester County, providing intensive case management and connection to housing, health, education, and community resources on site.

You will develop Individual Service Plans (ISPs), set achievable goals for stability and self-sufficiency, and meet bi-weekly with clients to monitor progress in a supportive team environment.

Qualifications

• BA or BS degree in social work, psychology or related field; extensive human services experience preferred but not required.
• Experience in case management and advocacy is desirable.

Responsibilities

• Provide intensive case management using a family-centered approach.
• Assist clients in achieving stability and housing, employment, self-sufficiency.
• Build connections to health, education, housing, and benefits resources.
• Develop Individual Service Plans (ISPs) with clear goals and action steps.
• Meet bi-weekly with clients to monitor progress and provide guidance.
