Care Plus Hospice is seeking a compassionate Registered Nurse Case Manager to join our West Valley team. You will coordinate hospice care, perform skilled nursing visits, and educate patients and families in home settings and facilities.

Join a collaborative interdisciplinary team and help ensure compliant, patient-centered care with a focus on dignity and comfort for end-of-life journeys in Arizona.

Qualifications

• Current Arizona Registered Nurse license in good standing.
• Minimum one year of nursing experience.
• Hospice or home health experience preferred.
• Strong assessment, communication and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Current CPR certification.
• Valid Arizona driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Manage and coordinate an assigned hospice patient caseload.
• Perform comprehensive nursing assessments.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing visits in patients' homes, assisted living facilities, and skilled nursing facilities.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, CNAs, and other interdisciplinary team members.
• Educate patients and families regarding disease progression, symptom management, medications, and hospice services.
• Participate in interdisciplinary team (IDT) meetings.
• Complete accurate and timely electronic documentation.
• Participate in on-call rotation as assigned.
• Ensure compliance with Medicare Conditions of Participation and hospice regulations.

Position Summary

The RN Case Manager is responsible for coordinating and managing hospice care for an assigned caseload of patients.
