Care Plus Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to serve the West Valley community. The role focuses on coordinating comprehensive hospice care, educating patients and families, and collaborating with an interdisciplinary team to deliver high-quality, patient-centered services.

The RN will conduct assessments, develop plans of care, and ensure compliance with Medicare requirements while providing skilled nursing visits in home and facility settings.

Qualifications

• Current Arizona RN license in good standing.
• Minimum one year of nursing experience.
• Hospice or home health experience preferred.
• CPR certification required or maintained

Responsibilities

• Coordinate and manage an assigned hospice patient caseload.
• Perform comprehensive nursing assessments.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing visits in patients' homes and facilities.
• Collaborate with interdisciplinary team members (IDT).
• Educate patients and families on disease progression, symptoms, medications, and hospice services.
• Participate in IDT meetings and care planning.
• Complete accurate electronic documentation in a timely manner.
• Participate in on-call rotation as assigned.

Position Summary

The RN Case Manager is responsible for coordinating and managing hospice care for an assigned caseload of patients. This role works closely with the interdisciplinary team to develop individualized plans of care, provide skilled nursing services, educate patients and families, and ensure quality care throughout the hospice journey.

Responsibilities

• Manage and coordinate an assigned hospice patient caseload.
• Perform comprehensive nursing assessments.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing visits in patients' homes, assisted living facilities, and skilled nursing facilities.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, CNAs, and other interdisciplinary team members.
• Educate patients and families regarding disease progression, symptom management, medications, and hospice services.
• Participate in interdisciplinary team (IDT) meetings.
• Complete accurate and timely electronic documentation.
• Participate in on-call rotation as assigned.
• Ensure compliance with Medicare Conditions of Participation and hospice regulations.

Qualifications
