Job summary

Hospice of the Valley is seeking an experienced RN Case Manager for Hospice Home Care in the far East Valley territory. This full-time, 40-hour/week day shift role coordinates the interdisciplinary team to deliver compassionate home-based hospice care.

Qualified candidates are graduates of accredited nursing programs with at least 1 year of medical/surgical or critical care experience, strong assessment and communication skills, and a dedication to patient- and family-centered care. EOE/M/F/D/V

• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.

Responsibilities

• Provide and coordinate hospice care for home care patients.
• Oversee the interdisciplinary hospice team to ensure patients' physical, emotional and spiritual needs are met.
• Educate patients and families about the disease process and symptom management.
• Ensure care delivery primarily takes place in the patient's home or in facilities where care is provided.

Position Profile

RN case managers have the primary responsibility to provide and coordinate hospice care for home care patients. They give care and oversee the interdisciplinary hospice team to make sure patients' physical, emotional and spiritual needs are met. RN case managers educate patients and families about the disease process, help manage and control symptoms and offer support and strength during difficult times.Care delivery primarily takes place in the patient's home or in a facility where the patient receives care.

• Facilitates smooth transition between care environments.


• Provides effective patient/family/caregiver/caregiver teaching.

• Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services.

Minimum Qualifications
