Job summary

Public Health Management Corporation is seeking a Case Manager for the Bridge Intensive Prevention Services in West Philadelphia. You will support at-risk youth (ages 10–19) and their families with trauma-informed, strengths-based case management, intake coordination, and home visits.

You will collaborate with a Therapist/Counselor, conduct school record reviews, assist with court appearances, and lead life skills groups while maintaining accurate documentation and program records.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or related field.
• At least two years of case management experience.
• Experience with trauma-informed care for at-risk youth and families.
• Ability to collaborate with diverse community partners.

Responsibilities

• Maintain a caseload of ~17 adolescents and families with weekly coordination.
• Collaborate with Therapist/Counselor to provide trauma-informed services.
• Schedule intake appointments and conduct home visits (min. 2 per month).
• Review school records monthly and accompany youth to court hearings.
• Submit monthly case updates to DHS, DA, and Philadelphia PD as needed.
• Lead life skills groups and educational sessions; educate on harm reduction.
• Participate in service plan meetings and build relationships with providers.
• Incorporate discharge planning from admission; attend trainings and meetings.
• Maintain CBPS records and perform administrative duties for program growth.
