Job summary

S:US in New York is seeking a Wellness Specialist to join the ACT Team, delivering wellness management services and coordinating linkage to health, housing, entitlements, education and vocational programs to support successful community placement.

You will provide individual and group services alongside a multidisciplinary team, participate in daily meetings, and assist with housing attainment, social relationships, and recovery from mental illness in a client-centered, trauma-informed approach.

Qualifications

• Bachelor's or Master’s degree in social work, psychology, rehabilitation counseling, or a related field.
• Two years of experience in human services with crisis assessment and treatment.
• Experience with individuals who have serious mental illness and/or co-occurring substance use disorders.

Responsibilities

• Provide wellness management services within the ACT framework.
• Lead groups and individual sessions; collaborate with the team for treatment planning.
• Link clients to health, housing, entitlement, educational, housing and vocational programs to support community placement.
