Job summary

S:US is seeking a Wellness Specialist to join the ACT Team, delivering evidence-based mental health treatment to individuals with severe mental illness and co-occurring substance use disorders. The role supports linkage to health, housing, entitlement, education and vocational programs to promote recovery and community placement.

Qualifications

• Excellent verbal, written and interpersonal communication skills.
• Proficient in computers; MS Word/Excel/Teams; Zoom/Webex.

Responsibilities

• Provides 80 percent of treatment contacts in the community and leads groups when clinically indicated.
• Ensures all documentation is completed timely and in compliance with agency and OMH regulations.
• Maintains progress notes and treatment plans with timely updates.
• Collaborates with the individual’s support network on treatment planning and case management.
• Responds to consumer needs promptly, accurately and with courtesy and respect.
