Job summary

S:US in New York is seeking a Wellness Coach to assist program participants with housing stabilization, access to services, and the development of a referral network. You will provide case management, develop service plans, accompany participants to appointments, advocate for clients, and coordinate with supportive services while documenting activities and participating in team supervision.

Qualifications

• Bachelor’s degree in Social Work or related human services field; two years of experience working with individuals with mental illness, homelessness, or in human services.

Responsibilities

• Provide case management to assigned caseload.
• Develop and monitor individual service plans in conjunction with participants.
• Assist program participants with problem solving and crisis intervention.
• Accompany participants to medical, psychiatric, and public benefit appointments.
• Advocate for program participants and support self-advocacy.
• Identify daily living skill problems and provide hands-on assistance or referral.
• Support facilities management to maintain apartment standards.
• Provide supportive counseling as needed.
• Conduct regular home and hospital visits as necessary.
• Document all services in compliance with agency standards.
• Collaborate with social service teams on activities and community inclusion.
• Liaison to community services and participate in supervision and trainings.
• On-call participation for crises and emergencies.
• Other duties as assigned.
