Job summary

Electra Hospital District seeks a Fitness Specialist to support chronic care management and wellness initiatives. You will design and supervise individualized exercise programs, conduct assessments, and coach participants toward healthy lifestyle goals.

Collaborating with CCM nurses and rehab staff, you will promote safe activity, track progress, and provide education to diverse ages across the lifespan while maintaining accurate documentation and compliance with safety policies.

Qualifications

• Knowledge of exercise physiology and fitness assessment methods.
• Ability to design evidence-based exercise programming for diverse populations.
• Strong coaching, motivational, and interpersonal skills.
• Excellent communication and customer service abilities.

Responsibilities

• Plan, develop, and supervise individualized exercise programs for CCM and wellness participants.
• Conduct fitness assessments to establish baselines and goals.
• Monitor progress and modify programs to ensure safety and goal achievement.
• Collaborate with CCM nurses, providers, rehab staff, and dietitians to support coordinated care plans.
• Provide exercise safety education and promote healthy lifestyle practices.
• Lead group classes, workshops, and wellness initiatives.
• Maintain accurate documentation of assessments, plans, attendance, and outcomes.
