Job summary

Bridgeway is seeking a Wellness Specialist to join a multi-disciplinary team at our Elizabeth, NJ location. You will provide on-site wellness and recovery-oriented services, case management, and linkage to community resources for individuals in crisis and those with co-occurring disorders.

Responsibilities include motivational engagement, WRAP/PAD development, education, and crisis de-escalation, with a strong emphasis on recovery-focused practice and collaboration with clinicians.

Qualifications

• BA/BS/BSW in a social service discipline is required.
• At least one year post-degree experience providing case management services for people with mental health and co-occurring disorders.
• CPRP certification preferred; discharged veterans given preference; new graduates will be considered.

Responsibilities

• Engage individuals into BCIS services using motivational approaches.
• Link with needed community services to divert from emergency services.
• Coordinate and maintain referral and community service lists.
• Provide case management activities as identified in the Treatment Plan.
• Develop Wellness Recovery Action Plans (WRAP) or Psychiatric Advanced Directives (PAD).
• Provide mental health education and medication education on-site.
