Job summary

Heartland Family Service in Omaha seeks an InSHAPE Health Mentor to lead staff, manage referrals, cultivate community partnerships, and plan group activities. The role requires delivering wellness coaching and facilitating group activities in community settings.

The position supports fitness, nutrition, and health education for members, maintains a smaller client load, and trains mentors. This is a full-time, on-site role in Omaha.

Qualifications

• Bachelor’s degree in health-related field required.
• Mental Health Experience preferred.
• Current CPR and First Aid Certifications.
• Valid driver’s license, acceptable driving record.

Responsibilities

• Provides community-based support services to insure implementation of client’s health goals.
• Manages a caseload of approximately 15 clients to establish and implement individual InSHAPE plans.
• Works collaboratively with clients, program staff and Program Director to design and support individual member plans.
• Provides fitness training, monitoring, continuing reassessment of exercise, nutrition, and healthcare commitments.
• Assists in training new mentors, interns, and volunteer mentors.
• Leads referral list management and weekly team meeting.
• Display a courteous and caring attitude at all times to Heartland Family Service clientele, volunteers, and visitors.

Mental Health Experience

Education

Bachelor’s Degree in Health Science, Social Work, Rehabilitation, Psychology, Health Promotion or related field

Personal Trainer Certification

Current CPR and First Aid Certifications

Valid driver’s license
