Job summary

Cascadia Health is seeking a Counselor III for Integrated Team-Based Care at Garlington Health Center in NE Portland. This in-person role focuses on behavioral health assessments, treatment planning, and coordinated care across disciplines. Schedule is Monday–Friday, 8:30 a.m.

to 5:00 p.m., with emphasis on trauma-informed, person-centered care and teamwork. The position requires a master’s degree in behavioral health, QMHP credentialing per Oregon rules, and ability to travel across Portland

Qualifications

• Master’s degree in a behavioral science field from an accredited college or university.
• Competencies to become credentialed as a QMHP per Oregon Administrative Rules.
• Must meet requirements to be credentialed as QMHP through MHACBO.

Responsibilities

• Conduct timely behavioral health assessments for new and ongoing clients.
• Perform DSM-5 diagnostic evaluations to inform treatment planning.
• Develop and implement treatment plans and regular reviews.
• Engage clients and support systems in person-centered service planning.
• Monitor progress and revise plans with interdisciplinary input.
• Provide individual and group counseling and community-based interventions.
• Coordinate with hospitals for discharge planning and continuity of care.
• Offer clinical guidance and support to team members and respond to crises.
