Job summary

Qualifications

- Supporting value-based care initiatives.
- Promoting patient engagement in wellness activities and screenings.
- Assisting primary care practices in improving quality metrics.
- Educating patients on disease prevention and health maintenance.
- Collaborating with healthcare providers to coordinate care.
- Performing clinical duties in compliance with policies.
- Participating in quality initiatives.
- Completing other duties as assigned.
