Job summary

Meritus is seeking a Wellness Registered Nurse (RN) for a full-time dayshift role in Maryland. Under physician supervision, you will deliver patient-centered care, assess and plan care, and support primary care practices with value-based outcomes.

This role emphasizes preventive care, patient engagement, and population health management, collaborating with care teams to identify high-risk patients and connect them with resources.

Qualifications

• ASN with 6 years clinical experience or BSN with 4 years clinical experience.
• Current Maryland RN license (or compact state) with primary source verification.
• Current BLS (CPR) required.

Responsibilities

• Provide preventive and wellness-focused nursing care and education.
• Support value-based care initiatives and MDPCP programs.
• Promote patient engagement in wellness activities and screenings.
• Assist practices in improving quality metrics and population health.
• Educate patients on disease prevention and healthcare utilization.
• Collaborate with clinicians and care teams to coordinate care.
• Perform duties per policies and regulatory requirements.
• Participate in performance improvement and quality initiatives.
• Complete other duties as assigned.
