Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse (PRN) to support residents with ongoing clinical monitoring and care coordination. The role involves assisting community nurses, coordinating admissions, and ensuring care aligns with plans while managing medications per orders.

The position emphasizes teamwork, flexibility, and a warm, encouraging demeanor in a senior living setting. RN or LPN licensure and geriatric experience are preferred.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license (LPN or RN).
• Three years’ experience in assessment and geriatric nursing preferred.

Responsibilities

• Assist in coordinating the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Coordinate care and services with outside providers including resident physicians.
• Document and report care provided and resident observations including significant changes.
• Maintain knowledge of state regulations, policies, and procedures.

Job Description

Salary: $31.40 - 36.90 per hour, based on experience

Schedule: PRN with the ability to pick up full shifts and/or the potential to support insulin management from 7a-9a and/or 4p-6p.

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans

Qualifications

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred
