Job summary

Spectrum Retirement Communities seeks a Wellness Nurse to support residents through clinical monitoring and team coordination. You will assist with admissions, ensure care plans are followed, and administer medications under physician orders.

The role requires a practical nursing credential, active licensure, and 3+ years in geriatric assessment. Knowledge of state regulations is essential. This is a full-time, on-site position with competitive benefits.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing.
• Knowledge of state regulations, policies, and procedures.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Assistance with coordinating care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.

Salary: $31.40-$36.00 per hour

Schedule: Full-Time, Wednesday-Saturday, 8:30am-6:00pm

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.
